Title: Bug: Output VCA2 "Note Hold" affects both Output VCAs, not just VCA2

I'm pretty sure this is a bug. If you set one of the Output VCA Release values to the maximum ("Note Hold"), it sets both Output VCAs to Note Hold.

Please fix this, as the two Output VCAs should really be acting completely independently.

And while I'm at it...having a "MIDI" option for the LFOs without a Re-Trigger option makes no sense. Being able to retrigger LFOs adds so many features...not just for the obvious typical EDM uses, but as clock dividers...don't underestimate the power of clock dividers in a modular. Especially when the Modular V sequencer does not allow you to turn the trigger for certain steps on/off (and Modular V doesn't allow applying more than one trigger type to a destination...)

A clock divider divides a clock pulse and outputs different divisions from different sockets for different rhythmic and timing events. Here's an example http://www.4mspedals.com/rcd.php
